How to Apply?

Steps:
  1. Download and fill (type only) application form
  2. Attach attested photocopies of requisite documents and recent photograph
  3. Deposit annual membership fee (payable as per BPS) in the FBR Foundation's Bank account (No. 4141556454 in National Bank of Pakistan, Super Market, F-6 Branch (Bank Code No. 0854), Islamabad)
  4. Attach attested copy of bank-deposit slip as proof of fee payment (kindly keep original deposit-slip of fee payment for your own record)
  5. Submit duly filled (typed) Membership form along with documents to the designated Focal Person for FBR Foundation, in your office
  6. All employees (serving/retired) are required to make full annual payment (as per their grades), before 30th June, 2017. The membership of FBR foundation will remain valid till 30th June, 2018

  1. Field Offices: Additional Commissioner (HQ)/Additional Collector (HQ)/Additional Director (HQ)
  2. FBR (HQ), Islamabad: Secretary Welfare, R# 539, 5th Floor FBR House, Islamabad
